Personal or Entity Funds: This contribution is made from the personal funds of the individual or the general treasury of the corporation, labor organization, or association named above, and is not being made by one person in the name of another.

U.S. Eligibility: I am a U.S. citizen or lawfully admitted permanent resident (i.e., green card holder), or this contribution is made by a business entity or organization domiciled in the United States.

State Contractor Prohibition (Pay-to-Play): I certify that neither I nor the entity making this contribution, nor any "affiliated person" or "affiliated entity" (as defined by 30 ILCS 500/50-37), is a state contractor or bidder with aggregate contracts/bids exceeding $50,000 annually that would prohibit this contribution under Section 50-37 of the Illinois Procurement Code.

Cook County Vendor Limitation: I certify that if I or the entity making this contribution has engaged in business with Cook County in excess of $10,000 within the past four years, this contribution will not cause total annual contributions to exceed the $1,500 calendar year limit established by Cook County Ethics Code Section 2-585.

No Public Funds: No part of this contribution is derived from public funds or the treasury of any unit of government.

Non-Deductibility: Contributions to JIM4HD17 PAC are not tax-deductible for federal or state income tax purposes .

2026 Contribution Limits: For the general election cycle,  individuals may contribute up to $7,300, and corporations, unions, or associations may contribute up to $14,600. PACs may contribute up to $72,800.

Reporting Disclosure: State law requires us to report the name and address of all contributors giving over $150 in a reporting period. For individuals giving over $500 in a calendar year, we must also report their occupation and employer.
